Health Butler provides screening and preventive health information derived from reliable sources such as the National Institutes of Health (NIH), the Centers for Disease Control and Prevention (CDC), and the U.S. Department of Health and Human Services (USDHHS). However, Health Butler IS NOT A HEALTHCARE PROVIDER. OUR INFORMATION IS NOT INTENDED TO REPLACE MEDICAL ADVICE FROM A HEALTHCARE PROFESSIONAL. ALWAYS CONSULT YOUR HEALTHCARE PROVIDER BEFORE MAKING ANY DECISIONS REGARDING YOUR OWN HEALTHCARE.

Health Butler provides health information and general preventive health guidelines but CANNOT GUARANTEE THE VALIDITY OF THESE GUIDELINES FOR ALL INDIVIDUALS. ALWAYS CONSULT YOUR HEALTHCARE PROVIDER BEFORE MAKING ANY DECISIONS REGARDING YOUR OWN HEALTHCARE.

Health Butler frequently updates its health information content in order to keep up with changes in national recommendations. Be aware that any and all information on the site is subject to change without notice. Health Butler is not liable for actions taken as a result of viewing content on the website. In addition, Health Butler reserves the right to make changes to the Terms and Conditions, Privacy Policy listed here without notice.

For use by those 18 years of age or older
The information and tools available on the Health Butler website are of greatest benefit to and are most accurate for individuals 18 years of age or older. Therefore, website use should be limited to those 18 years of age or older. Sections of the site contain information regarding sexual activity and sexual disease, so children should not access the site.

Registering on the Health Butler website
Registration for patients or individuals:
Health Butler offers storage of personal health information for individuals who want to register on the website. This information is used for customization of preventive health recommendations. Registration requires that an e-mail address be provided for communication and security purposes. Subscription fees are paid through the secure PayPal payment system, and identifying information that you provide PayPal is not stored at the Health Butler website (see below). Otherwise, no other personally identifiable information is stored for registered users. Health Butler does not employ spam mail and will not intentionally distribute registered users' e-mail addresses. Health Butler will not disclose, sell, distribute, or otherwise transfer registered user data in individual form to any third party without consent from the user, unless required by law. Health Butler provides e-mail reminders of personalized preventive care guidelines to registered users who wish to receive these reminders. HOWEVER, WE DO NOT GUARANTEE DELIVERY OF THESE E-MAIL REMINDERS. FURTHERMORE, HEALTH BUTLER RESERVES THE RIGHT TO DISCONTINUE ITS E-MAIL REMINDER SERVICE WITHOUT NOTICE. DO NOT RELY SOLELY ON HEALTH BUTLER FOR KEEPING TRACK OF YOUR PREVENTIVE HEALTH. Individuals choosing to input their Employer ID# to link to their employer's account consent to allowing the employer and associated parties to: 1) view aggregate, but NOT individual, preventive health data; 2) view a list of enrolled employee e-mail addresses and associated account expiration dates; and 3) elect to pay for or not pay for their employees' Health Butler accounts, as described below.

Registration for employers:
Employers are offered an independent registration process at the Health Butler for Employers subdomain. In order to confirm enrollment of employees, employers are provided a list of enrolled employees' e-mail addresses. Employers are also provided with aggregate preventive health data for their employees, but employers do not have access to individual or personally identifiable health information of their employees. Employers are free to utilize provided aggregate employee health data for internal purposes, but are strictly forbidden from distributing, selling, or otherwise transferring these data to any third party. Fees for employer accounts vary by company size and specifications. A price agreement between Health Butler and the employer must be negotiated prior to or concurrently with the employer enrollment period. For employers enrolled in Health Butler through their insurance broker, fees may be covered by the broker, in which case cost is negotiated independently with the insurance broker.

Registration for health insurance brokers:
Health insurance brokers are offered an independent registration process at the Health Butler for Brokers subdomain. Brokers can track company enrollment, but do not have access to individual or aggregate health information of company employees. Brokers have the option to pay for enrolled companies (clients). A price agreement between Health Butler and the broker must be negotiated prior to or concurrently with each employer enrollment period.

Registration for healthcare providers:
Healthcare providers are offered an independent registration process at the Health Butler for Providers subdomain. In order to confirm enrollment of patients, providers are provided a list of enrolled patients' e-mail addresses. Providers are also provided with aggregate preventive health data for their employees, but employers do not have access to individual or personally identifiable health information of their employees. Employers are free to utilize provided aggregate employee health data for internal purposes, but are strictly forbidden from distributing, selling, or otherwise transferring these data to any third party. Service is provided at no cost to healthcare providers, but patients must purchase their own accounts independently.

Subscription fees and PayPalTM
For patients or individuals:
Registering with Health Butler automatically enrolls the registering user in a free trial membership period. After this free trial period, subscription fees may apply, but only with the user's consent. Specific details about subscription fees and plans are available on the user's account pages. The duration, price, and terms of subscription plans are subject to change without notice. Fees must be paid in a timely manner in order to avoid account expiration. Health Butler uses the PayPalTM payment system for processing of subscription fees, and subscribing users are required to agree to Health Butler's policies (herein) as well as any and all of PayPal's terms and policies. Subscribing users may automatically be billed to maintain or renew subscriptions. Users wishing to cancel subscriptions must do so on their PayPal account. Health Butler is not responsible for any personal or financial information stored with PayPal.

Individuals electing to enroll in combination with an employer will either be billed directly or their employer will be billed for their account fees. This decision is at the employer's discretion. In the case of employees paying for their own accounts, appropriate fees apply.

For employers:
Employer accounts are billed on a monthly or annual basis at the price agreed upon prior to completion of the individual employer's enrollment period. Fees may be paid directly or via the PayPalTM payment system. Fees must be paid in a timely manner in order to avoid account expiration.

For brokers:
Broker accounts are billed on an annual basis at the price agreed upon prior to completion of each individual employer's enrollment period. Fees must be paid in a timely manner in order to avoid account expiration.

For providers:
Provider accounts are provided at no cost, but individual patient enrollees must purchase their own accounts.

Linking Health Butler and Google HealthTM Accounts
For Health Butler and Google HealthTM registered users only. Individuals registered with Health Butler have the option of linking to a Google HealthTM Account. Choosing to link the two separately registered accounts is optional and requires consent from the user. By linking the accounts, the user agrees to allow Health Butler to transfer his/her information to this 3rd party (Google HealthTM). CONSENT TO LINK THE TWO ACCOUNTS WILL AUTOMATICALLY INITIATE TRANSFER OF PERSONAL HEALTH INFORMATION BETWEEN HEALTH BUTLER AND GOOGLE HEALTHTM. HOWEVER, SUCCESSFUL TRANSFER OF THIS INFORMATION TO AND FROM GOOGLE HEALTHTM IS NOT GUARANTEED.

Once the two accounts have been linked, they will remain linked until the user opts to unlink the accounts. Opting to unlink the accounts will NOT result in deletion of any health information previously sent from the user's Health Butler account to the user's Google HealthTM account. Health Butler will automatically send updates of the individual user's Screening and Preventive History to his/her Google HealthTM Account, where the information will be posted under the "Immunizations" and/or "Procedures" and/or "Test Results" categories. HOWEVER, HEALTH BUTLER DOES NOT GUARANTEE THIS DELIVERY. FURTHERMORE, HEALTH BUTLER IS NOT RESPONSIBLE FOR ANY HEALTH INFORMATION STORED WITH GOOGLE HEALTHTM, INCLUDING BUT NOT LIMITED TO INFORMATION TRANSFERRED TO GOOGLE HEALTHTM FROM HEALTH BUTLER. Information may also be transfered from Google HealthTM to Health Butler, as seen on the user's "Update My Screening and Preventive History" page. HOWEVER, HEALTH BUTLER DOES NOT GUARANTEE THIS DELIVERY.

Unregistering
To unregister your account from Health Butler, please send an e-mail to requesting that your account be deleted. Please note that unregistering will result in deletion of all of your account information.

Use of website content
The health information provided on the Health Butler website may be printed, copied, and stored for personal use only. It may not be distributed or sold without expressed consent from Health Butler. Referencing the website content is permissible, but such references must include a link to the information source, the Health Butler name, and the title of the page from which the material is referenced.

Health Check and Personal Health Report Card
Unregistered users may enter in a few personal health details and generate a Health Check, which summarizes general screening and preventive health recommendations for individuals in their category. Registered users receive a similar but more detailed Personal Health Report Card. Both are generated using an algorithm that may not be accessed or utilized without expressed consent from Health Butler. The Health Check and Personal Health Report Card do not make medical recommendations but instead provide preventive information that should be discussed with a healthcare provider. For registered users, a Screenings/Immunizations Compliance Percentage is provided; however, recommendations can vary by individual so discuss all healthcare decisions with your healthcare provider. Enrolled employers can view aggregate employee reports generated from the same algorithms used to produce non-employer recommendations.

Health Habits Tracking
Registered users are provided Health Habits tracking tools for optional use. These tools are intended to provide the user the opportunity to track his/her health habits over time. A Health Habits Score is also provided and is based on the user's provided health habits information. HOWEVER, USERS SHOULD NOT RELY ON THEIR HEALTH HABITS SCORE AS A MEASURE OF OVERALL HEALTH. Health Butler does not account for all factors measuring overall health and recommendations can vary by individual, so consult your healthcare provider for more specific and detailed analysis and recommendations. Enrolled employers can view aggregate employee reports generated from the same algorithms used to produce non-employer recommendations.

For public use in the United States
Health Butler draws much of its information from organizations under the U.S. Federal Government, so use of the site is necessarily recommended only for individuals living in the United States.

Linking to Health Butler
It is permissible to link directly to the main Health Butler website and to specific sections of the site. All links to Health Butler should include the Health Butler name and a text-only link to the main site or a site section. Links must not include anything that may be degrading to the Health Butler organization, name, or website. Health Butler reserves the right to disallow linking to the site if any violation of these requirements is found.

Consent to contribute to aggregate statistical data
By agreeing to these terms, the user consents to allow Health Butler to compile aggregate data without identifying the user individually. Such data may be used for marketing purposes, to study the overall effect of Health Butler, and to assess trends among our users. Aggregate data may also be provided to sponsors.
